Before clearing a patient for a Skilled Nursing Facility (SNF) discharge, check that they have the proper pharmacy listed. Sending orders to a standard retail pharmacy instead of the facility’s contracted specialty distributor negatively affects patient care.
Remember - All controlled substances need new prescriptions sent to the SNF pharmacy, even if they are being continued from prior to admission!
