Death Certificates

1. Navigate to https://okta.loginmt.com/ and login
2. Scroll down and select HHS APP-VERS
3. Select Certify MI -> click the green >> button beside the appropriate name
4. Shelly Boyd, if present, will review the patient’s chart and fill in the relevant details prior to routing to you. Mark yourself as the “Certifying Physician” and check for accuracy.
5. Review for accuracy or add fields as needed if Shelly hasn’t or is on vacation.
6. Final Step: Click PRESS TO CERTIFY button.
